Multiple Symptom questionnaire (MSQ)

Self assessments are key to understanding the way you feel. I use an MSQ on every intake, and if the numbers are high, weekly updates show us how quickly symptoms can be reduced with simple, nutritional changes.

Here are a few of the areas to be tracked:

Headaches __________ Faintness __________ Dizziness __________ Insomnia Total _________ __________ Watery or itchy eyes __________ Swollen, reddened or sticky eyelids __________ Bags or dark circles under eyes __________ Blurred or tunnel vision

Total _________

Itchy ears __________ Earaches, ear infections __________ Drainage from ear __________ Ringing in ears, hearing loss

Total _________ __________ Stuffy nose __________ Sinus problems __________ Hay fever __________ Sneezing attacks __________ Excessive mucus formation

Total ________

Each area relates to a specific part of our digestive tract, so we can determine where support is needed with dietary changes. As you reduce inflammatory foods, your numbers will come down. Let me know if you would like to see the other 12 sections of the MSQ!
